In accordance with the Honor Code at Hampden-Sydney College, your signature below certifies that all information and supporting materials - are my own work, factually true, and honestly presented, and that these documents will become the property of Hampden-Sydney College and will not be returned to me. I understand that I may be subject to a range of disciplinary actions, including admission revocation, expulsion, or revocation of course credit, grades, and degree should the information I have certified be false.

I agree to notify Hampden-Sydney's office of admission immediately should there be any change to the information requested in this application including disciplinary history.

I acknowledge that every school I have attended may release all requested records and recommendations to Hampden-Sydney including conduct/disciplinary records. I also authorize employees at Hampden-Sydney to confidentially contact my current and former schools should they have questions about the information submitted on my behalf.

I acknowledge that I have received the application instructions for Hampden-Sydney. I understand that all offers of admission are conditional, pending receipt of final transcripts showing work comparable in quality to that upon which the offer was based. Any dismissal or leave taken from school will automatically cause a revocation of admission to Hampden-Sydney pending a thorough review of such action by the Dean of Admission at Hampden-Sydney.
